1. What is CHG’s Dividend Policy?
The Company has a dividend policy on paying dividend not less than 50 percent of Net Profit from the company’s consolidated financial statement, after deducting the legal reserve and other reserves. However, dividend payment is subject to change depend on its performance, business expansion plan, financial status, Liquidity and any necessities in the future. In the past 3 years, the Company paid the dividend more than that specified on the dividend policy as shown on the table below.

Please contact Thailand Securities Depository Company Limited (TSD) at 02-229-2888, fill out TSD-101 Form and attach the securities certificates duly signed by both the transferor and the transferee together with the copies of National ID cards of both the transferor and the transferee. In case that the copies of National ID cards are not available, the document certified by the sub-district or district office can be used.

6. What should I do if I lost my securities certificate?
Please contact Thailand Securities Depository Company Limited (TSD) at 02-229-2888 and fill out TSD-102 form together with the supported document as follows :
- A copy of complaint form filed at Police Station mentioning the shareholder name, the serial numbers of shares and total number of shares.
Note : You can get all these information from Thailand Securities Depository Company Limited (TSD). - A copy of the national ID card of the shareholder, duly signed.
- In case that the copy of National ID card is not available, the document certified by the sub-district or district office can be used.
7. In the event that the shareholder has passed away, what should be done?
Shareholders’ heirs have to submit their request to the court to appoint an estate administrator. Upon receiving the Court Order, Estate administrator should contact Thailand Securities Depository Company Limited (TSD) at 02-229-2888 and submit the required document as follows
- A TSD-103 Administration of Estate Form, duly signed by the estate administrator.
- A copy of Court Order appointing the estate administrator, duly signed by the estate administrator on all pages.
- A copy of Death Certificate, duly signed by the estate administrator.
- A copies of National ID and House Registration of the estate administrator, duly signed.
- A copies of National ID and House Registration of the heir, duly signed.
- In case that National ID Cards of the estate administrator and the heir are not available, the document certified by the sub-district or district Office can be used.
- The Original Securities Certificates duly signed by the estate administrator and the heir.
8. How can I change the detail of my shareholder information?
You can change your title, name, surname or mailing address by contacting Thailand Securities Depository Company Limited (TSD) at 02-229-2888, fill out TSD-301 Amending Securities Holder Records Form and attach the copies of your National ID card duly signed.

9. If I wish to have dividends credited to my deposit account, what should I do?
Please contact Thailand Securities Depository Company Limited (TSD) at 02-229-2888, fill out TSD-202 Dividend/Interest Payment to Bank Account Form and attach the copies of your National ID card and the front page of your bank passbook duly signed.

10. When does CHG report its Financial Statement?
- Quarterly financial statements: within 45 days from the end of each quarter.
- Annual financial statements: within 60 days from the end of an accounting period.
11. How can I purchase and sell CHG share?
Chularat Hospital Group (CHG) is a listed company on Stock Exchange of Thailand (SET). Therefore, you can purchase and sell CHG shares through any Security Companies.

13. In case that I can’t join an Annual General Meeting, what I should do?
You can grant your authorization to your representative by filling out the Letter of Attorney, attach the copies of your National ID Card and your representative duly signed and submit them to the Company before joining an Annual General Meeting.
